WAIVER AND RELEASE OF LIABILITY

2026 DREAMERS MEN'S BASKETBALL TRYOUT

READ CAREFULLY BEFORE SIGNING

1. Acknowledgment of Risks

I, the undersigned participant, acknowledge that I have voluntarily chosen to participate in the 2026 DREAMERS Mens Basketball Tryout (the “Tryout”). I understand and agree that participation in any sports activity, including basketball, involves inherent risks, dangers, and hazards that may cause serious injury, including, but not limited to, sprains, fractures, concussions, and, in rare cases, paralysis or death. These risks may result from the actions, omissions, or negligence of others, including other participants, coaches, or staff, as well as from the physical condition of the playing facilities and equipment. I knowingly and voluntarily accept and assume all risks of injury, whether caused by the negligence of the DREAMERS organization, its coaches, staff, volunteers, or otherwise.

2. Medical Condition

I certify that I am in good physical condition and have no medical or physical conditions that would prevent me from participating in the Tryout. I understand the importance of seeking professional medical advice before engaging in any strenuous physical activity. I acknowledge that I am responsible for my own medical treatment should an injury occur, and I agree to bear all associated medical expenses.

3. Release and Waiver of Claims

In consideration for being permitted to participate in the Tryout, I hereby release, waive, and discharge the DREAMERS organization, its officers, employees, coaches, agents, and volunteers (collectively, the “Released Parties”) from any and all claims, demands, or causes of action of any kind whatsoever arising from my participation in the Tryout. This release includes, without limitation, any and all claims for personal injury, property damage, or wrongful death, whether caused by the negligence of the Released Parties or otherwise.

4. Indemnification and Hold Harmless

I agree to indemnify, defend, and hold harmless the Released Parties from any and all losses, damages, liabilities, costs, and expenses (including reasonable attorneys' fees) incurred as a result of my participation in the Tryout, including, but not limited to, any claims brought by third parties arising from my conduct or actions.

5. Permission to Use Likeness

I grant permission to the DREAMERS organization to use any photographs, videos, or other recordings of me taken during the Tryout for promotional and marketing purposes without compensation.

6. Severability

I agree that if any portion of this Waiver and Release of Liability is held to be invalid, the remaining portions shall remain in full force and effect.

7. Governing Law

This agreement shall be governed by and construed in accordance with the laws of the State of [Insert State Here].

8. Acknowledgement

I have read this document carefully and understand all of its terms. I have had the opportunity to ask questions and have received satisfactory answers. I am executing this Waiver and Release of Liability freely and voluntarily, and I intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
